ONLINE DATABASE UPDATE BY PARENTS/GUARDIANS
Dear Parents/Guardians
We are currently consolidating our payments, memberships, attendance and student database into a new system that will be more effective. So, please bear with us if your membership cards/renewals are held up.
Meanwhile, we would like your assistance by filling up your current particulars into our online database by using our new online registration form. We would kindly appreciate your fastest response by Wed 29 Feb 2012, so as to assist us in ensuring a smooth transition to our new database before the start of term 2.
Procedures for updating the new database/registration forms
One registration form for each family. A family consists of:
1. Parents/Guardians of children enrolled in our classes.
- Please note that the family name is the child's last name.
- Details of Parent/Guardian #1 should be the primary contact and Parent/Guardian #2, the secondary point of contact.
- Although in our form, it will not be presented as a required field, please ensure that accurate cellphone numbers of both parents/guardians are included.
- The primary contact will be the number we send SMS notifications/reminders on classes and events, including billing information.
- For all Singapore contact numbers, please start with "+65" followed by the phone number with no spaces between, eg +6512345678.
- In an emergency, if we are not able to contact Parent/Guardian #1, we will contact Parent/Guardian #2.
2. Children enrolled in our classes.
- If more than one child is currently registered with the studio, please ensure that the details of each child is included before submission of the form.
- If you have 3 children and only 2 are enrolled in classes, please do not input the details of the child who is not currently enrolled in our Dance / Circus programme.
3. Parents/Guardians who are also current students of our adult classes.
- If you (the parent/guardian) are also doing classes at the studio, please fill in your details as a student.
Please provide accurate information - inaccurate and insufficient information will delay the updating process.
Once you submit the information, you will not be able to make any changes.
- Any future changes will only be made via email to dance@swingapore.com.
